Database with web form on top. What's the best way to do this? (Event management)

Not sure if this is the best way to get this information, but wasn't sure of another way.
I'm looking for a method to manage events. Basically, to be able to research an event, and identify ones that we want to have a presence at. So need to be able to manage things like location, name, contact info, notes, whether insurance is required from the vendor, and attaching pictures.
Thought we could build this fairly easily if we could find a database service that allows us to build our own custom webform to tie to it. I don't mind paying, but eventually am hoping to have several individuals connecting to it to both add information as well as review.
This is for a non-profit, so cost is important. Not opposed to using a NON SaaS solution, that I can host in AWS or Azure on a low-grade server.
